This tip sheet assists navigation staff in supporting clients with serious mental illness (SMI) in their management of diabetes and augments a navigator’s understanding of the standards of care for diabetes amongst the SMI population. Additionally, this tip sheet encourages consideration of specific physical and social needs of individuals with SMI and diabetes and signals specific warning signs that indicate the need to engage clinical staff.
