The My Health, My Life Toolkit is a toolkit for individuals with intellectual and developmental disabilities (I/DD), their family member(s) or guardian(s) and their provider support team.
The purpose of the My Health, My Life Toolkit is to:
- Improve communication when the individual enters and leaves the hospital or caresetting; and
- Build or strengthen relationships between medical providers and the individual and his or her family member(s) or guardian(s).

The four tools in the My Health, My Life Toolkit include:
Personal Health Record
The individual and his or her family member(s) or guardian(s) should review and update the Personal Health Record regularly and bring it with him or her to all appointments and care settings. The Personal Health Record includes important personal information, additional needs and instructions, emergency contact information, provider contact information, health conditions, hospital visits, an appointment planner and medication records.
Hospital Companion Guide
The Hospital Companion Guide helps prepare the individual and his or her family member(s) or guardian(s) for the transition home from the hospital or other care setting. The guide includes valuable checklists to make sure the individual and his or her family member(s) or guardian(s) understand the changes in support after leaving the hospital or care setting.
Discharge Planning Tool for Individuals with I/DD
The individual’s provider support team (e.g., doctors, nurses, care managers, discharge planners, social workers or other providers) should use the Discharge Planning Tool to help support the transition home from the hospital.
Self-Advocacy Empowerment Tool
The individual and his or her family member(s) or guardian(s) should use the Self-Advocacy Empowerment Tool to help plan for services and supports as needs, preferences, and interests evolve over time.
