As part of the “Individuals with Intellectual and Developmental Disabilities (I/DD) Webinar Series,” this webinar provides an overview on the aging and I/DD networks and how to leverage them. The purpose of the session is to encourage innovative cross-network coalitions, centered on Aging and Disability Resource Centers (ADRCs), to develop Transition Care Plans and Comprehensive Care Coordination Programs incorporating strategies for breaking down the “silos of care” separating the LTSS and health-related networks in leveraging services for the aging I/DD population.
