The second webinar in this two-part series focuses on how behavioral health organizations and providers can provide navigation services to their clients with serious mental illness (SMI) and common chronic physical health conditions. The webinar explores components of navigation services including personalized health coaching and wellness planning as well as actions navigators can take to link these individuals and their families to essential health and community services. The webinar features six condition-specific navigation tip sheets that highlight considerations related to the management of SMI and other physical conditions including diabetes, hypertension, congestive heart failure, HIV/AIDS, and COPD and asthma and smoking cessation.

Presenters highlight strategies that were implemented to successfully integrate navigation services within their organizations. Following the webinar, participants should be able to:

  1. Understand key functions and services that navigators can provide;
  2. Identify tangible steps to support individuals with SMI and chronic physical health conditions; and
  3. Facilitate conversations with clients on managing their chronic conditions and mental illness, using the RIC navigation tip sheets.
