This spotlight describes how Partners Health Plan (PHP), New York’s first and only Medicare-Medicaid plan that exclusively serves individuals with intellectual and developmental disabilities (I/DD), implements a person-centered and integrated approach to care planning and service delivery that begins with a commitment to the whole person. Specifically, this spotlight:

  • Highlights PHP’s assessment and care planning tools and methods for training staff to ensure that care coordinators have the capabilities and skillsets necessary for supporting the unique needs and preferences of members with co-occurring I/DD and behavioral health needs.
  • Outlines how PHP fosters strategic partnerships to successfully build a comprehensive network of providers and community-based organizations for integrated access to behavioral health services.
