Person & Family Engagement

Proactively engaging dually eligible beneficiaries, and their families and caregivers, as partners in care planning and delivery is critical to understanding and responding to individual preferences and needs and improving quality and outcomes of care. The framework of person and family engagement (PFE) involves understanding “patients and families as partners in defining, designing, participating in and assessing the care practices and systems that serve them to assure they are respectful of and responsive to individual patient preferences, needs, and values.”1

Dually eligible beneficiaries represent diverse backgrounds and experiences, and have varied needs and preferences. Resources for Integrated Care (RIC) features resources for providers and plans serving dually eligible individuals that are interested in strengthening PFE approaches, with an emphasis on:

  • Addressing social determinants of health (SDOH),
  • Motivational interviewing,
  • Person-centered care, and
  • Self-management support.
