This is the first webinar in the eight-part series “Strategies for the Implementation of Disability-Competent Care”.
This webinar discusses the paradigm shift from a medical model to the independent living/person-centered model that is integral to the delivery of disability-competent care, and explores the importance of new terminology (for example, from care plan to life plan). This webinar introduces the building blocks of the disability-competent model of care, including: 1) relational care management delivered by an interdisciplinary team, 2) highly responsive primary care, and 3) redesigned and flexible long term services and supports.
