The CMS Innovation Center invites you to participate in a listening session, Person-Centered Listening Session: Leveraging Relationships with Community-Based Organizations to Meet Health-Related Social Needs on April 24, 2024. This event will feature Innovation Center leadership and other experts who will explore how community-based organizations are being leveraged to provide support to beneficiaries in meeting health-related social needs and share their impact on patient experience navigating care.

We look forward to gathering the insights from this session as it will help inform the Center’s strategy across its portfolio and throughout the lifecycles of its models.
