What is Self-Management Support?

  • Self-management support is a collaborative process between providers and consumers that consist of activities that help individuals manage, cope, and live with a chronic illness and participate in and self-direct care.
  • The concept of self-management is inclusive of health promotion and wellness, whereby individuals engage in a wide range of behaviors to maintain their health (e.g., diet, exercise, sleep, stress reduction).
