The following two guides can help your health plan understand the resources that are available in your community and establish linkages with community partners.

Community Resources Guide – This guide provides brief descriptions, examples, and national directories of these services and resources.  Your plan may customize this guide by adding details that are relevant to your unique member population or the specific communities you serve.
