A strong IDT is necessary for providing care coordination that keeps the participant’s experience and concerns at the forefront of care delivery. This presentation includes practical and functional approaches to developing, managing, and executing IDTs. The webinar examines a highly integrated provider and plan IDT, while also exploring strategies to build and support partnerships with local community-based practices. Presenters provide concrete and innovative strategies to improve the management and communication within the IDT, including meeting execution, agenda setting, and facilitating communication when in-person meetings are not practical.
