Join the Partnership to Align Social Care for an upcoming discussion with officials from the U.S. Department of Health and Human Services (HHS), as well as key community and health care leaders on opportunities to address health-related social needs (HRSNs) through community partnerships. This panel discussion will spotlight the Call to Action: Addressing Health-Related Social Needs in Communities Across the Nation released by HHS to catalyze community-level partnerships and call upon leaders in health care, social services, public health, and health information technology to work together to address HRSNs and create a stronger, more integrated health and social care system through shared decision making.
The activities outlined in the Call to Action align closely with the goal of the Partnership to Align Social Care to engage stakeholders across sectors to co-design and enable alignment between health care organizations and community care networks to create an equitable, efficient, sustainable, and person-centered health and social care ecosystem.
Specifically, participants will:
- Hear from HHS leaders about the federal priorities and the whole-of-government approach toward fostering opportunities for health care, social services, and other important stakeholders to work together across silos to address HRSNs;
- Get detail on specific commitments from community and health care leaders aimed at promoting and achieving key opportunities outlined in the Call to Action; and
- Gain information about emerging and ongoing opportunities to adopt leading practices to contribute to and sustain momentum toward meaningfully aligning health and social care.
