Description:
What are the hallmarks of person-centered care? How can health plans and providers respect the needs, values, and preferences of beneficiaries dually eligible for Medicare and Medicaid while providing high-quality, seamless care? How can providers and patients work together to achieve the best outcomes possible? This webinar examines three important strategies for achieving person-centered care:
- Goal-driven care, in which treatment decisions are founded on an enrollee’s expressed goals and success is measured by the extent to which these are attained;
- Shared decision-making, a collaborative process that allows individuals and their providers to make health care decisions together, taking into account the best scientific evidence available, as well as the enrollee’s values and preferences; and
- Supported decision-making, a process in which adults who need assistance with decision-making receive the help they need to understand the situations and choices they face, so they can make life decisions for themselves, without undue or overbroad guardianship.
