This tip sheet is intended for health plan leaders and providers who are responsible for organizing and coordinating the completion of the Disability Competent Care Self-Assessment Tool (DCCAT). The DCCAT was developed to help health plans evaluate their current ability to meet the needs of adults with functional limitations and to identify strategic opportunities for improvement. The tips are based on the experience of the participants in the Disability Competent Care Learning Community in taking the self-assessment and using the results to identify an area of focus for their work to improve care for their participants with disabilities.
