Tool: Disability-Competent Care SBAR

The SBAR (Situation, Background, Assessment, and Recommendation) is a quality improvement tool that can be used by health plans and providers as they work together to improve their communication and coordination of care for those with disabilities.  It was used by care coordinators within the Disability Competent Care Learning Community to improve the coordination and communication of information about participant needs and preferences to primary care practitioners as part of an intervention to have care coordinators attend primary care visits with their participants (i.e., a co-visit).
